Project Screening Request Form

Note that due to the volume of requests we receive, please expect a response in four to six weeks.

If you are completing Part 1 of either the Short or Long Environmental Assessment Form (EAF), instead of submitting a request to NYNHP, use the online EAF Mapper (https://gisservices.dec.ny.gov/eafmapper/). Also use the EAF Mapper if you only need to know the rare species you are required by state regulation to address. The EAF Mapper will provide you with the same information as a report from NYNHP.

If you would also like information on any rare plants and animals near your project site that are not listed by NYS, and for which there are no regulatory mandates, before submitting a request to NYNHP please consult the online Environmental Resource Mapper (ERM) (https://gisservices.dec.ny.gov/gis/erm/); if the information provided by the ERM does not meet your needs, then you are welcome to submit a request to NYNHP. Please see Request Natural Heritage Information on the DEC website (https://www.dec.ny.gov/animals/31181.html) for more details about the EAF Mapper and the ERM. Using these online resources can preclude the need to submit a request to NY Natural Heritage.

To request a review of a specific project site, please complete and submit this form (but see exceptions below). The New York Natural Heritage Program will review our database of locations of rare species and significant natural communities, and we will report records which are in the vicinity of the project site and for which impacts from the project or action may need to be assessed. NYNHP does not have the authority to comment on permit requirements or potential impacts, nor can we give official clearance to any project on behalf of NYS.

We recommend that you submit a location map of your project site as a .pdf file as part of this form. Please do not submit architectural or engineering renderings or photographs.
If you do not have a map in .pdf format, you must provide at least two of the following: tax parcel number, latitude/longitude (if available, coordinates are very helpful), and/or street address including city.

Exceptions: Do not use this form if your project
• is more than 1000 acres in size
• is a linear project such as a transmission line or pipeline more than 2 miles in length
• includes parts of three or more counties.
or otherwise has a complicated or complex footprint (e.g., multiple sites; doesn't follow tax parcel boundaries) that would be difficult to map in GIS from a static map.
For these types of projects, please send a request and a GIS (shapefile or .gdb) file of your project site directly to the New York Natural Heritage Program
at NaturalHeritage@dec.ny.gov.

If your company, organization, or agency is not the project sponsor or applicant, and you are submitting this request on behalf of the project applicant (e.g., a consulting firm submitting on behalf of a client), you may enter the name of the project applicant here.

Briefly describe the proposed project or activity, including why you need information (e.g., SEQR review, environmental assessment for permit, planning board approval, management plan). If the proposed action is undergoing SEQR review, also include the name of the lead agency. Example: “A 25 unit residential single-family housing development is proposed for an 150-acre parcel. The development will occupy 100 acres; the remaining 50 acres will be left undisturbed. We have been contracted to review the environmental impacts of the project.”
Briefly describe the current land use and habitats at the project site (e.g., wooded, agricultural, developed commercial). Example: “The site is currently undeveloped, and mostly wooded with some former agricultural fields.”
Please attach one or two maps of your project site as .pdf files. The map should include names of nearby roads and other landmarks, and the boundary of the proposed project site should be clearly labeled, marked or highlighted. You may submit more than one map if they show the project site at different scales or with different backgrounds. Please do not send architectural or engineering drawings or photographs. If you are not submitting a map, you must complete at least two of the following: Tax parcel number, Latitude/Longitude, and Street Address of Project Site.
